Overview

Sehat Card Plus enables eligible patients to receive kidney transplant free of cost at designated hospitals

Hospitals Providing Kidney Transplant Services

– Institute of Kidney Diseases (IKD), Peshawar
– MMC General Hospital, Peshawar
– Rehman Medical Institute (RMI), Peshawar

Important Notes for Patients

– For kidney matching, all screening, tests, and other expenses before the operation will be the responsibility of the patient.
– The patient is responsible for arranging the kidney donor as well as obtaining the No Objection Certificate (NOC) by the relevant transplant regulatory authority Khyber Pakhtunkhwa Medical Transplantation Regulatory Authority (KPMTRA)

Process

1. Consult the doctor, at any of the above-mentioned hospitals.
2. After medical evaluation, visit State Life, Sehat Card Plus Office, 1st Floor, State Life Building, 34 The Mall, Peshawar Cantt. with the following documents for registration:
– Complete test and screening reports
– Donor details
– NOC
– CNICs of both patient and donor

Cost Coverage by Sehat Card

– Full cost of the kidney transplant procedure
– Post-transplant medications for one year

مریضوں کے لیے اہم ہدایات

گردہ میچنگ لے لیے آپریشن سے پھلے مریض اور ڈونر کے تمام سیکرنگ، ٹیسٹ اور دیگر اخراجات مریض کے ذمہ ہونگی۔

مریض خود گردہ عطیہ کرنے والے (ڈونر) کا انتظام کرے گا اور متعلقہ خیبر پختونخوا امیڈیکل ٹرانسپلانٹ ریگولیٹری اتھارٹی سے این او سی حاصل
کرنا بھی مریض کی ذمہ داری ہوگی۔

اوپر دیے گئے کسی بھی اسپتال میں ڈاکٹر سے مشورہ کریں۔

طبی معانئے ہے بعد درج ذیل دستاویزات کے ساتھ اسٹیت لائف، صحت کارڈ دفتر (پہلی منزل،اسٹیٹ لائف بلڈنگ، 34 دی مال، پشاور کینٹ) میں رجسٹریشن کے لئے تشریف لے جائیں۔
