Application Portal for Hospital Empanelment is open now
We are pleased to announce that the portal for hospital empanelment process is now open. Hospitals interested in registering with the Sehat Card Plus KP Program can apply through online application form available at www.statehealth.com.pk
The portal will remain open until July 15th, 2024.

ہم تمام ہسپتالوں کو مطلع کرتے ہیں کہ ہسپتالوں کی ایمپینلمنٹ کا آن لائن درخواست پورٹل اب کھل گیا ہے۔
ہیلتھ کارڈ پلس پروگرام میں شمولیت کی دلچسپی رکھنے والے ہسپتال اپنی درخواستیں ہماری ویب سائٹ(www.statehealth.com.pk) پر آن لائن جمع کروا سکتے ہیں۔
پورٹل 15 جولائی تک کھلا رہے گا۔

March 10, 2025No there is no separate card. Present patient’s CNIC on Health Card Counter at our empaneled hospital and avail Health Card Services.
